Submit bids in a sealed envelope to the bid box at Central Supply Chain Management Directorate (Old Boys School Building), 310 Jabu Ndlovu Street, Pietermaritzburg, 3200 by 04 June 2026 at 11:00. Late bids will not be accepted. Use official bid forms (SBD series) without re-typing. Photocopies are allowed but must bear original signatures. Each bid must be in a separate envelope with bidder’s name, address, bid number, and closing date clearly marked. Bids sent via post, fax, or electronic means will not be considered. All pages must be initialed. Correcting fluid is prohibited. Bids will be opened in public after closing time. Proof of posting is not accepted as proof of delivery.

Evaluation will use the 80/20 or 90/10 preference point system based on the bid value. Points are allocated as follows: 80 or 90 points for price (using the formula Ps = 80(1−(Pt−Pmin)/Pmin) or Ps = 90(1−(Pt−Pmin)/Pmin)), and 20 or 10 points for specific goals. Specific goals include: 20 points for companies at least 100% Black African-owned, or at least 51% Black People (women)-owned, or at least 51% Black People/Persons with Disabilities-owned. Bidders must submit proof of claims for preference points. Failure to submit required documentation will result in zero points for specific goals. The lowest acceptable bid determines the applicable system for non-income-generating contracts.

Supply, delivery, installation, commissioning, and maintenance of a theatre table orthopaedic spinal electronically operated (M38B) for Ngwelezane Hospital. Offers must strictly comply with specifications; exceeding requirements is acceptable. Certificates of Compliance (SABS/SANS/ISO) must be submitted for items referencing these standards. Radiation Control License numbers must be provided for equipment listed in the Hazardous Substances schedule. Technicians must be original equipment manufacturer-trained; proof of training must be submitted. Quality of services must not be less than specified. No guarantee is given on quantities; orders will be placed on an 'as and when required' basis.

Pricing must be inclusive of all applicable taxes (e.g., VAT). Bid prices must be submitted in South African Rand. Payment terms: invoices must be original, labeled 'INVOICE' (non-VAT) or 'TAX INVOICE' (VAT), and include VAT number for VAT vendors. Invoices must contain: supplier name/address/registration, recipient details, serialized number, date, description/quantity of goods/services, value of supply, tax amount, and consideration. Contractor is responsible for statutory increases or foreign exchange fluctuations. No additional costs will be covered for these. VAT registration is compulsory if taxable supplies exceed or are likely to exceed R1 million in any 12-month period. Contractor must ensure tax compliance throughout the contract period.

CSD registration is mandatory; failure to register by closing time disqualifies the bidder. Tax Compliance Status (TCS) PIN or CSD number must be submitted. Bidders must ensure compliance with tax obligations; TCS certificates can be submitted with the bid. Consortia/Joint Ventures must submit separate TCS/CSD numbers for each party. B-BBEE: 20 preference points for companies at least 100% Black African-owned, or at least 51% Black People (women)-owned, or at least 51% Black People/Persons with Disabilities-owned. EMEs/QSEs must complete the sworn affidavit for B-BBEE exemption. Joint Ventures must submit a consolidated B-BBEE certificate and separate certificates for each member. No bids will be considered from state employees or entities with state-employed directors/members. Bidders must disclose any relationship with KZN Department of Health employees via SBD4 form. Foreign suppliers must comply with NIP Programme if imported content exceeds USD 10 million (or equivalent thresholds).
