• SBD 1 (Invitation to Bid): bidder and contact details, signed as the offer cover page.
• SBD 4 (Declaration of Interest): discloses whether any director or shareholder is employed by the state or connected to anyone at the procuring institution.
• SBD 6.1 (Preference Points Claim): claims B-BBEE points under the PPPFA 80/20 system.
• Evidence of CSD registration (MAAA number) and tax compliance status (SARS PIN if non-compliant).

Disqualification risks:

Evaluation split: 80 points for price, 20 points for specific goals (80/20 system).

Price points calculated using the formula Ps = 80 x (1 - (Pt - Pmin) / Pmin).

Specific goals points (max 20) awarded per the table below, based on ownership percentages:

• Black Ownership: 51-100% = 10 points; 30-50% = 6; 5-29% = 4; 0-4% = 0.
• Black Women Ownership: 51-100% = 4; 30-50% = 2; 5-29% = 1; 0-4% = 0.
• Black Youth Ownership: 30-100% = 3; 5-29% = 2; 0-4% = 0.
• Disability: 30-100% = 3; 5-29% = 2; 0-4% = 0.

Proof for specific goals: B-BBEE certificate or affidavit, CSD report, ID documents of owners, municipal account or lease agreement, doctor's certificate or disability database.

Failure to submit proof with the quotation means no specific goal points are claimed.

Supply and delivery of bulk buying (clothing) for the Uphongolo Municipality area. Quotations must be per quotation numbers.

Three quotation packages:

• Package 1 – Thandiwe Nkosi (School Uniforms & Accessories): beanies (70 each of navy/sky and navy/gold), school bags (20), school shoes Toughees (20), jerseys (35 each for Tholulwazi, Umbekwa, Mnyame, Dingukwazi), socks (35 each for four schools), pants (20 each navy, black, grey), ties (35 each for four schools).
• Package 2 – Lindiwe Banda (High School Wear): jerseys (10 each in black/gold, green/white, royal/white, green/gold, sky/white), skirts (10 each green, maroon, navy), pants (10 navy trouser).
• Package 3 – Cynthia Mthethwa (Fabrics & Sewing Raw Materials): peacock tetrex fabric (210m), maroon tetrex (70m), bottle green tetrex (70m), navy tetrex (70m), tracksuit navy fabric polyester (70m), tracksuit powder blue fabric polyester (70m), needles size 14 & 15 (40), fabric scissors heavy duty (10), Elemkey tool/set (3), zips (500), roll Vialene white interfacing (2).

Quantities and specifications strictly follow the OVF Beneficiary Bulk Buying Procurement Plan.

Pricing schedule instructions: Bidders must complete all highlighted unit rate and total price columns (Excl. VAT and Incl. VAT). Prices must remain firm for 90 days from tender closing date. Calculations for 15% VAT and Grand Totals must be accurate; incomplete pricing schedules lead to disqualification. Quantities and specifications follow the OVF Beneficiary Bulk Buying Procurement Plan. Fixed price quotations on company letterhead required; price must be inclusive of VAT and all disbursement costs.

Pricing schedule: complete all highlighted unit rate and total price columns (Excl. VAT and Incl. VAT).

Prices must remain firm for 90 days from the tender closing date.

Calculations for 15% VAT and Grand Totals must be accurate; incomplete pricing schedules lead to disqualification.

Master procurement recapitulation: provide total bid price for each package and cumulative grand total.

CSD registration: must be registered on the National Treasury Central Supplier Database (MAAA) and provide the CSD number.

Tax compliance: provide SARS Tax Compliance Status (TCS) PIN; if non-compliant, supply the SARS PIN. A printed TCS certificate may be submitted. Each party in a consortium/JV/subcontractor must submit a separate TCS certificate/PIN/CSD number.

B-BBEE: a valid B-BBEE certificate or sworn affidavit (for EMEs/QSEs) must be submitted to claim preference points.

State employment: bids from persons in the service of the state, or companies/close corporations with directors/members in the service of the state, will not be considered.

SBD 6.1 declaration: certifies that specific goal points claimed are true and correct; fraudulent claims may lead to disqualification, contract cancellation, restriction from doing business with the state for up to 10 years, and criminal prosecution.

Specific goals points are awarded based on ownership percentages: Black Ownership (51-100%: 10 points, 30-50%: 6, 5-29%: 4, 0-4%: 0), Black Women Ownership (51-100%: 4, 30-50%: 2, 5-29%: 1, 0-4%: 0), Black Youth Ownership (30-100%: 3, 5-29%: 2, 0-4%: 0), Disability (30-100%: 3, 5-29%: 2, 0-4%: 0). Total specific goals points: 20. Proof may include B-BBEE certificate or affidavit, CSD report, ID documents of owners, municipal account or lease agreement, doctor's certificate or disability database.

The successful bidder will be required to sign a written contract form (SBD 7). The bid is subject to the Preferential Procurement Policy Framework Act, 2000, the Preferential Procurement Regulations, 2017, the General Conditions of Contract (GCC), and any special conditions of contract. Fraudulent claims for specific goals may lead to disqualification, recovery of costs, contract cancellation, restriction from state business for up to 10 years, and criminal prosecution.

Mandatory requirements: Signed and completed SBD 1, 4 and 6.1; evidence of registration on the National Treasury Central Supplier Database (MAAA) with confirmation of Tax Compliance Status (SARS Pin to be supplied if Tax Status non-compliant). Failure to provide any mandatory information will result in the submission being deemed non-responsive.

Quotations will be evaluated based on an 80/20 price/specific goals allocation - 80 points for price and 20 points for specific goals. Fixed price quotations on company letterhead are required; price must be inclusive of VAT and all costs relating to disbursements.
