• SBD 1 (Invitation to Bid): bidder and contact details, signed as the offer cover page.
• SBD 4 (Declaration of Interest): discloses whether any director or shareholder is employed by the state or connected to anyone at the procuring institution.
• SBD 6.1 (Preference Points Claim): claims preference points for specific goals under the 80/20 system.
• CSD registration number (MAAA) and SARS Tax Compliance Status PIN (or TCS certificate).
• Completed pricing schedule (Annexure A) with unit rates and totals.

Disqualification risks:

• Any mandatory document missing, unsigned or incomplete.
• Quotations received after the closing time.
• Bids from persons in the service of the state, or companies/close corporations with directors/members in the service of the state.
• Bidders listed on the Register for Tender Defaulters or the List of Restricted Suppliers.
• Failure to submit on the official forms or in the prescribed manner.

Evaluation split: 80 points for price, 20 points for specific goals (80/20 system).

Price points: calculated using the formula Ps = 80(1 - (Pt - Pmin)/Pmin), where Pt is the tender price and Pmin is the lowest acceptable tender price.

Specific goals points (max 20):

• Black Ownership: 51%-100% = 10, 30%-50% = 6, 5%-29% = 4, 0%-4% = 0.
• Black Women Ownership: 51%-100% = 4, 30%-50% = 2, 5%-29% = 1, 0%-4% = 0.
• Black Youth Ownership: 30%-100% = 3, 5%-29% = 2, 0%-4% = 0.
• Disability: 30%-100% = 3, 5%-29% = 2, 0%-4% = 0.

Proof for claiming specific goals: B-BBEE certificate or affidavit, CSD report, ID documents of owners, municipal account or lease agreement, doctor's certificate or disability database.

Failure to submit proof with the bid means no preference points are claimed for that goal.

The organ of state may require substantiation of any claim at any time.

Supply and deliver shipping containers for the uMgungundlovu District Municipality area. Quotations must be per quotation number.

Six separate quotations, each for a different beneficiary and delivery location:

• Quotation 1: 20ft shipping container (water and wind tight, Grade A; exterior 6m x 2.4m x 2.6m; internal 5.9m x 2.35m x 2.39m) delivered to Inzinga - Impendle.
• Quotation 2: Same 20ft container delivered to Bruntville - Mooi River.
• Quotation 3: Same 20ft container delivered to No.2 Ebebhuzi Location - Mkhambathini.
• Quotation 4: 40ft shipping container (water and wind tight, Grade A; exterior 12.19m L x 2.44m W x 2.99m H; internal 12.025m L x 2.352m W x 2.585m H; usage capacity 76m3) delivered to Thokozani Location - Crammond.
• Quotation 5: 20ft storage container (same dimensions as 20ft) delivered to Kwa-Nyavu Area, Ebubhuze, Emkhambathini.
• Quotation 6: 6m shipping container (same dimensions as 20ft) delivered to Cabazini Area, Mooi River.

Each quotation includes delivery and offloading services.

Containers must be water and wind tight, Grade A.

Pricing schedule (Annexure A) must be completed with unit rates and totals for each item, in ZAR excluding and including 15% VAT.

Prices must be fixed and inclusive of VAT and all costs.

The schedule must be signed and submitted with the required statutory documents.

Fixed price quotations on company letterhead, inclusive of VAT and all costs relating to disbursements.

Prices must be quoted in South African Rand (ZAR), showing amounts excluding and including 15% VAT.

Complete unit rates and totals for each item in the pricing schedule.

Total bid price including VAT must be stated.

Offer valid for 90 days from the closing date.

CSD registration: Must be registered on the National Treasury Central Supplier Database (CSD) and provide the MAAA number.

Tax compliance: Must provide SARS Tax Compliance Status PIN (or TCS certificate). If tax status is non-compliant, supply the SARS PIN.

B-BBEE: Must submit a valid B-BBEE certificate or sworn affidavit (for EMEs/QSEs) to claim preference points.

No bids from persons in the service of the state, or companies/close corporations with directors/members in the service of the state.

Bidders must not be listed on the Register for Tender Defaulters or the List of Restricted Suppliers.

The bid is subject to the Preferential Procurement Policy Framework Act, 2000, the Preferential Procurement Regulations, 2017, and the General Conditions of Contract (GCC).

The successful bidder must sign a written contract form (SBD 7).

The contract is subject to the Preferential Procurement Policy Framework Act, 2000, the Preferential Procurement Regulations, 2017, the General Conditions of Contract (GCC), and any other special conditions of contract.

Fraudulent claims for specific goals may lead to disqualification, recovery of costs, cancellation of contract, restriction from doing business with the state for up to 10 years, and criminal prosecution.

Offer must be valid for 90 days from the closing date. The successful bidder must sign a written contract form (SBD 7).

Mandatory requirements: signed and completed SBD 1, SBD 4 and SBD 6.1; evidence of CSD registration (MAAA number) with confirmation of tax compliance status (SARS PIN if non-compliant). Failure to provide any mandatory information renders the submission non-responsive.

Evaluation is based on an 80/20 price/specific goals allocation: 80 points for price, 20 points for specific goals. Specific goals points are awarded per the table: Black Ownership (max 10), Black Women Ownership (max 4), Black Youth Ownership (max 3), Disability (max 3). Proof of B-BBEE certificate or affidavit is required for preference points.
