Sbd 6.1 Preference points claim form in terms of the preferential procurement regulations, 2022

The tender is for the supply of connection cables to connect monitors to laptops: 1.8m display port to VGA male cable.

Returnable form: SBD 6.1 (Preference Points Claim Form) must be completed, signed, and submitted with the quotation. The form must be accompanied by proof for each specific goal claimed (e.g., certified ID, company registration, CSD report, B-BBEE certificate or sworn affidavit, medical certificate for disability).

Preference points system: 80/20 system applies (price 80 points, specific goals 20 points). Specific goals and maximum points: Black ownership 10, Woman ownership 5, Disabled ownership 2, Youth ownership 3. Points for specific goals are claimed only if supporting documentation is submitted with the bid; failure to submit proof means no points are claimed for those goals. The organ of state may require substantiation of any claim at any time.

SBD 6.1 (Preference Points Claim Form) must be completed and signed. Bidders claiming specific goals must provide proof: for Black, Woman, or Youth ownership – certified ID, company registration document, CSD full detail report, and certified B-BBEE certificate or sworn affidavit; for Disabled – medical certificate from a practitioner registered with HPCSA. Fraudulent claims may lead to disqualification, recovery of costs, contract cancellation, restriction from doing business with organs of state for up to 10 years, and criminal prosecution.

Quotations will be evaluated utilising the 80/20 scoring system, where:

All quotations must be emailed to [email protected]. Quotations received after the closing date and time will not be accepted. Suppliers must be registered on the Central Supplier Database (CSD). Required documents: quotation with reference number, validity, delivery period, CSD Master registration number (MAAA) and tax compliance status PIN; completed SBD 4 and SBD 6.1 forms; e-filing copy of valid Tax Clearance certificate; certified copy of ID document (not older than 6 months); certified copy of company registration (CIPC) (not older than 6 months); certified copy of BBBEE certificate (not older than 6 months); where applicable, certified copy of OEM certification documents. All certified copies must be dated within 6 months of the quotation closing date; older or missing certification will be rejected and the supplier will score zero for that specific goal.

Quotations will be evaluated utilising the 80/20 scoring system: 80 points for price and 20 points for specific goals. Specific goals and points: Race (Black) 10 points, Women 5 points, Disabled 2 points, Youth (Below 35 years) 3 points. The appointment will not necessarily be based on the lowest quotation. Stats SA reserves the right to negotiate prices, award partially, not to make an award, and not to procure certain items. Stats SA may request additional information; failure to comply with the timeframe may render the quotation non-responsive.

All quoted prices should be valid (firm) for the duration of the order. Prices dependent on exchange rate should factor in price escalations. Stats SA will not provide upfront payment; payment will be made upon receipt of goods/services and within 30 days of receiving a valid invoice. Quotes should be on company letterhead, indicating VAT registration number if VAT registered. Quotes should be detailed and specify services and cost; market related prices will be negotiated with the preferred service provider.

Suppliers must be registered on the Central Supplier Database (CSD). Completed SBD 4 (Declaration of Interest) and SBD 6.1 (Preference Points Claim) forms must be filled in, signed and returned with the quotation. Failure to complete the declaration of interest form may result in disqualification. Certified copies of ID, CIPC, and BBBEE certificate or Sworn Affidavit must be dated within 6 months of the quotation closing date. Any document with certification older than 6 months or missing certification date, stamp entirely and signature will be rejected as invalid and the supplier will automatically lose points and score zero for that specific goal.
