Scope: Repair, test and commission a 900 mm diameter HDPE pipe at Protea Glen Ext 11 (Avalon Depot).

• Must comply with Johannesburg Water Safety, Health & Environmental (SHE) Specification and all applicable legislation.
• Appointments required: qualified First Aider, Incident Investigator, Emergency Coordinator, Firefighter, Risk Assessor, Hand Tools Inspector, Excavation Supervisor, Flagman, Confined Space Supervisor, Lifting Machinery Inspector (registered with Dept of Employment & Labour), etc.
• Insurance: valid Letter of Good Standing from compensation commissioner throughout the contract.
• Provide SHE documentation: baseline risk assessment, safe work procedures, incident management plan, emergency preparedness plan, PPE register, tool inspection registers, lifting equipment inspection reports, excavation supervision, confined‑space procedures, pressure equipment certificates, fall protection plan, etc.
• All safety files must be submitted for approval before work starts and returned on project completion.
• No employee accommodation on site; provide required welfare facilities (toilets, showers, changing rooms, drinking water).
• All personnel must wear SABS‑approved PPE and undergo inductions and weekly toolbox talks.
• Specific equipment inspections (hand tools, lifting machines, excavation shoring, confined‑space ventilation, pressure equipment) must be documented and signed off by competent persons.

Required compliance documents:

• Valid Tax Clearance Certificate (SARS PIN)
• Valid B‑BBEE certificate or sworn affidavit (must meet B‑BBEE Practice Guide)
• Municipal rates and taxes (not older than 90 days in arrears)
• Signed Declaration of Interest (MBD 4)
• Declaration of past supply‑chain management practices (MBD 8)
• Certificate of Independent Proposal Determination (MBD 9)
• Proof of CSD registration/MAAA supplier number
• Preference points claim form (MBD 6.1)
• Company registration documents with ID copies of directors/shareholders
• Appointments and competency certificates for construction supervisor, lifting machinery inspector, safety officer, etc.
• All documents must be original, valid and submitted as part of the e‑tender package.
