Do not retype the official forms; attach supporting certificates and other documents as annexures. - Keep all prices and details legible. Do not use correction fluid; cross out mistakes in ink and add a full signature at every alteration. - The bid must be received and deposited by 12:00 on friday, 20 november 2026. Mandatory supporting documents - original receipt for the bid document. - Covering letter. - Certified company or close-corporation registration certificate. - Certified identity document for a sole proprietor. - Compliant csd registration and a csd report NO older than 14 days before closing. - Original valid SARS tax clearance certificate and original tax compliance status pin; certified copies are not accepted. Each consortium, joint venture or subcontractor must submit its own pair. - Original or certified b-bbee status-level verification certificate; an eme must submit verification evidence. - Qcto or relevant seta accreditation, programme-approval and good-standing evidence covering every qualification offered, together with course content and the relevant industry registration where applicable. - A comprehensive company profile and detailed account of previous work may be added but are not compulsory. Mandatory forms - section 3 bid declaration: confirms the offer, pricing, participation in competing offers and acceptance of the stated conditions. - Board, member or partner resolution: authorises the named signatory and must be signed by all directors, members or partners, as applicable. - Consortium or joint-venture resolution: authorises the signatory and records the parties' joint and several liability. - Mbd 1 (invitation to bid): records bidder and contact particulars, VAT number, total price, number of items, south african representative status and the signed offer; missing particulars may result in disqualification. - Mbd 2 (tax clearance requirements): links the bid to the original tax clearance certificate and tax compliance status pin requirements. - Mbd 3.1 (Pricing schedule – firm prices): provides the three-year pricing, delivery information and any deviations. - Mbd 4 (declaration of interest): discloses any state-service status or relationships with state officials involved in evaluation or adjudication. - Mbd 5 (declaration for procurement above r10 million): addresses financial history and compliance where the procurement exceeds r10 million. - Mbd 6.1 (Preference points claim): claims price and specific-goal preference points; an unsupported claim receives NO points, while a fraudulent claim may lead to disqualification, cancellation, damages and exclusion from state business for up to 10 years. - Mbd 8 (declaration of bidder's past supply chain management practices): declares past supply-chain practices. - Mbd 9 (certificate of independent bid determination): confirms independent pricing without collusion or disclosure to competitors. - Previous work experience of a similar nature section: records comparable work undertaken during the last five years and supplies supporting references. Key rejection risks - late or non-physical delivery; removal or fragmentation of the official bid document; unsigned or incomplete schedules; illegible pricing; unauthorised corrections; or omitted tax, csd, b-bbee or accreditation evidence. - NO bid is accepted from a person in the service of the state.
