Scope: completion of outstanding works at Logobate Clinic (also referred to as Logobate CHC) for the Department of Health, Northern Cape Province.

Relevant previous experience on completed projects of a similar nature and value in the last 10 years (30 points, minimum 17). Only construction/renovation of buildings experience is considered. Signed and stamped client references for the same projects (20 points, minimum 11) or signed and contactable reference letters. Qualifications, skills and experience of key project resources (15 points, minimum 8). Supporting documents: particulars of tenderer's projects (appointment letters and completion certificates), schedule of references, CVs of key personnel including OHS, certified copies of certificates/qualifications (not older than 6 months), copies of professional registrations, and signed and stamped banking/financial documentation.

Pricing format: Bill of Quantities (BOQ) – each line item must be priced in black ink; the BOQ must be submitted in its entirety with no omissions or unauthorised alterations; non-applicable items must be marked 'N/A'.

• Valid letter of good standing from Department of Labour (COIDA) or FEM or RMA certificate. For JV/consortium, all partners must submit.
• CIPC registration (Company Incorporation Property Commission) with proof of verification. For JV/consortium, each company must be registered.
• CSD registration with MAAA number for verification. For JV, consolidated CSD number; for consortium, each partner must be registered.
• Attendance at the compulsory site briefing and signing the attendance register.
• Proof of active professional registration with SACPCMP for the construction manager (PrCPM or PrCM) for the project duration.
• Proof of active professional registration with SACPCMP for the health and safety officer (CHSO, CHSM or CHSA).
• Completed and signed Compulsory Enterprise Questionnaire (each JV/consortium member).
• Tax compliance: mandatory for award. The recommended bidder must provide a Tax Compliance Letter with unique PIN. If non-compliant, the bidder has 7 working days to provide written proof from SARS of compliance or an arrangement to meet outstanding obligations; failure results in rejection.
