Evaluation is a 2-stage process: Stage 1 Mandatory Requirements (valid COC for undertakers issued by eThekwini Municipality Health Unit, and premises meeting site inspection requirements in Section 10, Annexure A); Stage 2 Price and Preference. The 80/20 or 90/10 preference point system will apply, determined by the price offered by the lowest acceptable tender. Price points calculated per PPPFA Regulations. Preference points for specific goals: Ownership (Black ownership) weighting 50%, and RDP Goal (promotion of South African owned enterprises) weighting 50%, with points allocated as per tables. Bidders must be registered on the National Treasury CSD at closing; Joint Venture parties must each be registered. Bidders must submit a valid SARS Tax Compliance Status PIN or original Tax Clearance Certificate. Bidders must sign the Declaration of Municipal Fees confirming municipal fees are in order or that acknowledgement of debt agreements have been concluded. No tender will be considered from persons in the service of the state as defined in Regulation 44 of the Local Government: Municipal Supply Chain Management Regulations.

Provision of interment, cremation, exhumation and assisted burial for indigent persons within the eThekwini Municipal area for a period of thirty-six (36) months. Bidders may be required to state where samples of the full range of products can be inspected or to submit samples for inspection prior to the closing date. The names of the manufacturers of the goods or equipment offered must be stated in the tender. Bidders who are not manufacturers, accredited distributors, or agents must provide a valid agreement / Joint Venture. All goods are to be supplied only against the form of order issued by the Municipality. Bidders shall quote a unit price which includes delivery to the specified delivery point. Risk remains with the Supplier until goods are duly delivered. Bidders shall state the period within which delivery will be made after receipt of the official order.

All prices quoted in South African Rand, with VAT stated separately. Bidders may submit firm prices for each 12-month period. Estimated quantities are set out in Section 8: Bill of Quantities / Schedule of Rates/Activities. The Supplier will be bound to supply whatever quantities the Municipality may require. Bidders would be precluded if their pricing structure deviates from the Official Tender Form. The Head: Supply Chain Management reserves the right to require evidence of adequate financial standing.

The Tenderer's CSD Supplier Number (starting with 'MAAA') must be provided. Bidders must hold a valid certificate of competency (COC) for undertakers issued by the eThekwini Municipality Health Unit. Premises must meet site inspection requirements in Section 10, Annexure A. Bidders must submit a valid SARS Tax Compliance Status PIN or original Tax Clearance Certificate. Bidders must sign the Declaration of Municipal Fees confirming municipal fees are in order or that acknowledgement of debt agreements have been concluded. Bidders must complete and submit MBD 4, MBD 5 (if procurement above R10 million), MBD 6.1, MBD 8, and MBD 9. Bidders must sign a Declaration with respect to the Occupational Health and Safety Act, accepting the undertaking to perform the work / supply / services in compliance with the Act and the Employer's / Purchaser's / Client's Health and Safety Specifications.

The successful tenderer will be required to fill in and sign a written Contract Form (MBD 7). The Municipality does not bind itself to accept the lowest or any tender, and reserves the right to accept the whole or any part. Bidders shall not bind the Municipality to any minimum quantity. The successful tenderer shall be bound to provide any quantities stipulated in the specification. The Municipality may negotiate final terms with preferred bidders, provided no second or unfair opportunity, no detriment to other tenderers, and no higher price than the tender as submitted. The Municipality may cancel the tender at any time before award. Payment will be made only to the Supplier; factoring arrangements will not be accepted. Appeals may be lodged within 14 days of the decision or action, in writing to the Municipality.

The premises of the undertaker must meet all site inspection requirements provided in Section 10, Annexure A.
