Returnable documents (all must be completed, signed, and submitted in the same order as issued):

• MBD 1 (Invitation to Bid): bidder details and signed offer cover page.
• MBD 4 (Declaration of Interest): discloses any connection to state employees.
• MBD 6.1 (Preference Points Claim): claims B-BBEE and specific goal points.
• MBD 8 (Declaration of Past SCM Practices): declares no abuse of supply chain systems.
• MBD 9 (Certificate of Independent Bid Determination): certifies no collusion.
• Signed General Conditions of Contract (GCC).
• POPIA consent and acknowledgment form.
• Authority to Sign / Board Resolution authorising the signatory.
• Completed pricing schedule.

Additional requirements:

• All alterations must be initialled/signed by the authorised signatory or accompanied by a letter on official letterhead.

Disqualification risks:

Evaluation uses the 80/20 preference point system: 80 points for price and 20 points for specific goals.

Specific goals (each worth 10 points, verified via CSD):

• Empowerment of black persons: at least 51% black ownership.
• Enterprises with a workshop located within Gauteng Province.

Points for specific goals are not evaluated at the panel formation stage; they are allocated when quotations are requested during the contract validity period.

MBD 5 (Declaration for Procurement Above R10 Million) must be completed if the contract value exceeds R10 million (all taxes included). Mandatory registrations and documents:

• Tax Compliance Status (TCS) PIN or CSD number; each party in a JV/consortium must submit separately.

• CIDB registration where applicable.
• ID copies of shareholders/directors and share certificates (not older than three months).
• Signed JV agreement for joint ventures, with separate CSD registration and consolidated B-BBEE certificate.
