Bids must be submitted on the official forms provided, not re-typed. The successful bidder will be required to sign a written contract form (SBD 7). Returnable forms include SBD 1 (Invitation to Bid), SBD 4 (Declaration of Interest), SBD 6.1 (Preference Points Claim), and proof of authority to sign (e.g. company resolution). All supplier information and disclosure forms must be completed, signed and stamped by a Commissioner of Oaths. Failure to provide or comply with any required particulars may render the bid invalid.
