Quotes and accompanying documentation must be emailed to [email protected], [email protected] and [email protected]. Quotes must be valid for 60 days from quote date. Any alteration to the RFQ document other than completing bidder details and tendered price may render the submission non-responsive. Returnable documents: CSD report with supplier number, company details and tax status verification pin; detailed pricing structure on company letterhead inclusive of VAT, disbursements and escalations if applicable; valid B-BBEE certificate or sworn affidavit; signed RFQ document; Annexure A (SBD 4 – Bidder's Disclosure); Annexure B (SBD 6.1 – Preference Points Claim Form). Failure to supply any of the administrative documents may lead to the quote not being considered.

Administrative documents required: 1. Central Supplier Database report with supplier number, company details and tax status verification pin. 2. Detailed pricing structure on company letterhead, inclusive of VAT, disbursements and escalations if applicable. 3. Valid B-BBEE certificate or Sworn affidavit. 4. Signed RFQ document. Note: Failure to supply any of the administrative documents stipulated may lead to the quote not being considered.

Evaluation uses the 80/20 preference point system. Only bidders that pass mandatory compliance proceed to price and specific goals evaluation. Price points: 80; specific goals points: 20. Specific goals include B-BBEE status level contribution. Points for black ownership: 50% or more = 20 points, less than 50% = 10 points, 0% = 0 points. The lowest or only quotation will not necessarily be accepted. Iziko reserves the right to accept or reject any quotation and to withdraw the RFQ at any time before award.

Supply of Agilent High Sensitivity D1000 ScreenTape Assay and Agilent High Sensitivity D1000 Reagents for the Agilent Tapestation System. Item 1: High Sensitivity D1000 ScreenTape Assay (quantity 1) – DNA sizing range 35–1000 bp, sensitivity 5 pg/μL, quantitative range 10–1,000 pg/μL, quantitative accuracy ±20%. Item 2: High Sensitivity D1000 Reagents pack (includes ladder and sample buffer) (quantity 1) – DNA sizing range 35–1,000 bp, sensitivity 5 pg/μL, quantitative range 10–1,000 pg/μL, quantitative accuracy ±20%, sample volume 2 μL, kit capacity 112 samples/kit.

Pricing schedule: Item 1: High Sensitivity D1000 ScreenTape Assay (quantity 1) – price including VAT. Item 2: High Sensitivity D1000 Reagents pack (includes ladder and sample buffer) (quantity 1) – price including VAT. Item 3: Total evaluated bid price including VAT (sum of items 1 to 2). Pricing notes: Total evaluated bid price used for price evaluation under 80/20 preference point system; standardized provisional items included for fair comparison; no additional costs unless disclosed and approved in writing; bidders must attach breakdown of assumptions, resource categories and rates.

Item 3: Total evaluated bid price including VAT (sum of items 1 to 2). The total evaluated bid price is used for price evaluation under the 80/20 preference point system. No additional costs, disbursements or incidental charges will be paid unless disclosed in the bid and approved in writing by Iziko before being incurred. Bidders must attach a breakdown of assumptions, resource categories and rates used to prepare their pricing.

CSD registration: Central Supplier Database report with supplier number, company details and tax status verification pin. B-BBEE: Valid B-BBEE certificate or sworn affidavit. SBD 4 (Bidder's Disclosure): discloses whether any director or shareholder is employed by the state or connected to anyone at the department; false declarations lead to disqualification. SBD 6.1 (Preference Points Claim Form): claims preference points for specific goals; must be completed and signed. Signed RFQ document. Failure to supply any of the administrative documents may lead to the quote not being considered.

Preferential procurement: Proposals evaluated using the 80/20 preference point system. Specific goals include B-BBEE status level contribution. Points allocated: 80 for price, 20 for specific goals. Specific goals table: Proof of B-BBEE certificate, Company Registration, Identification Documentation, CSD report. Points for black ownership: 50% or more black ownership = 20 points, less than 50% = 10 points, 0% = 0 points.

The proposal and appended documentation may form the basis of an agreement to be negotiated and concluded in a formal contract between Iziko and the preferred service provider. A mere offer and acceptance shall not constitute a formal contract until the appropriate written agreement/project charter has been concluded. Any alteration to the RFQ document other than completing bidder details and tendered price may render the submission non-responsive. False declarations in the SBD 6.1 form may lead to disqualification, recovery of costs, cancellation of contract, restriction from doing business with the state for up to 10 years, and criminal prosecution.
