Request for quotation: appointment of a qualified service provider for the provision of maintenance of standby generator for period of twelve months at nhbrc sunninghill office

Returnable forms — all must be completed, signed and submitted with the quotation:

• SBD 1 (Invitation to Bid): provides bidder and contact details, signed as the offer cover page.
• SBD 4 (Declaration of Interest): discloses whether any director or shareholder is employed by the state or connected to anyone at the department; false declaration disqualifies the bid.
• SBD 6.1 (Preference Schedule): claims B‑BBEE preference points under the 80/20 system.
• SBD 7.2 (Contract Form – Rendering of Services): binds the service provider to the contract terms.
• MBD7 (Contract Form): the written contract required for the successful bidder.
• Proof of CSD registration and a valid SARS tax compliance status (TCS PIN or CSD number).
• B‑BBEE status level verification certificate or sworn affidavit (for EMEs/QSEs).
• COIDA certification, CIDB grading, qualified personnel and reference letters as specified.

Submission – quotations must be emailed to [email protected] by 18 September 2026 at 11:00 AM; late bids are rejected. All forms must be submitted on the official forms (not re‑typed). Attendance at the compulsory briefing on 09 September 2026 at 10:00 AM is required.

Bidders must submit a valid COIDA certificate (Electrical or Mechanical discipline) issued by the Department of Labour. A CIDB grading of 1ME or 1EB is required. Qualified generator technicians or diesel mechanics with trade certificates and CVs showing 3–5 years experience, and an electrician with relevant qualifications and wireman's licence (2–5 years experience) must be provided. Three contactable reference letters from clients for similar work in the last five years, on client letterhead, signed, with contact person and project start/end dates. Bidders must complete and submit all required SBD forms (SBD 1, SBD 4, SBD 6.1, SBD 7.2, etc.), provide a valid SARS Tax Compliance Status (TCS PIN or CSD number), and a B-BBEE status level verification certificate or sworn affidavit (for EMEs/QSEs). Bidders must not be in the service of the state and must declare any conflicts of interest. Foreign suppliers must complete the pre-award questionnaire.
