The SABS invites suitably qualified service providers to provide condom sampling and related services for a period of 12 months. The scope of services is detailed in Annexure A.

Returnable documents — all must be completed, signed and submitted with the quotation:

• CSD registration report from the National Treasury Central Supplier Database.
• Valid B-BBEE certificate or sworn affidavit (to claim specific goals).
• SBD 4 (Bidder's Disclosure): discloses any relationship with state employees or the procuring institution; false declarations disqualify the bid.
• SBD 6.1 (Preference Points Claim): claims specific goals points under the 80/20 system.
• Non-disclosure agreement: must be signed and returned.

Disqualification risks:

• Contacting any SABS personnel other than the named contact regarding this RFQ.
• Any canvassing by bidders or suppliers.
• Failure to provide or comply with any of the required particulars may render the bid invalid.

Functionality evaluation (minimum threshold 75%):

• Company track record (25%): reference letters for sampling at international level in the past 12 months. Scoring: 0 for none, 1 for 1 letter, 2 for 2, 3 for 3, 4 for 4, 5 for 5 or more.
• Booking confirmation (25%): written confirmation of capability to conduct sampling at the requested country within specified timelines. Scoring: 0 for no confirmation or >144 hrs, 1 for =144 hrs, 2 for ≥120 and <144 hrs, 3 for ≥96 and <120 hrs, 4 for ≥48 and <96 hrs, 5 for <48 hrs.
• Geographic coverage (25%): footprint in Africa, Asia, South America & Europe. Scoring: 0 for none, 1 for 1 continent, 2 for 2, 3 for 3, 4 for all four, 5 for all regions including others.
• Turnaround time (25%): from sampling to lab delivery, demonstrating logistical management (customs, clearing, shipping). Scoring: 0 for none or >14 days, 1 for 14 days, 2 for 13, 3 for 11-12, 4 for 9-10, 5 for 7-8 days.

Only bidders meeting the 75% threshold proceed to the 80/20 preference point system (price and specific goals).

Tax compliance: must provide a SARS Tax Compliance Status (TCS) PIN or CSD number; each party in a consortium/joint venture must submit separate TCS/CSD.

B-BBEE: valid B-BBEE certificate or sworn affidavit required to claim specific goals.

SBD 4 (Bidder's Disclosure): must be completed and signed.

SBD 6.1 (Preference Points Claim): must be completed to claim specific goals.

Non-disclosure agreement: must be signed.

State employment: bids from persons in service of the state, or companies with directors who are, will not be considered.

Foreign suppliers: must complete the questionnaire in SBD 1.

Contractual terms:

• The RFQ does not commit SABS to any course of action; SABS may reject any submission or discontinue the process at any time without liability.
• The quotation must remain valid for 120 days from closing date; SABS may extend this by up to 60 days by written notice without consent, and bidders may not amend their quotations during validity or extension.
• SABS will not be bound by any legal or contractual terms included in the bidder's quotation.
• The successful bidder will be required to sign a written contract form (SBD 7).
• Confidentiality: all information provided is covered by the non-disclosure agreement to be signed.

Special conditions:

• No advance payment will be made; payment only upon delivery of service or goods.
• SABS is not responsible for any costs incurred by the bidder in preparing or submitting the quotation.
• SABS will not be bound by any legal or contractual terms included in the bidder's quotation.

Evaluation criteria: functionality is assessed on company track record, booking confirmation, geographic coverage, and turnaround time, each weighted 25%. Minimum threshold is 75%. Only bidders meeting this threshold are evaluated on the 80/20 preference point system.
