SBD 6.1 (Preference Points Claim Form): Must be completed and submitted with the bid. Claims for specific goals require supporting documentation: certified ID, company registration, CSD full detail report, certified B-BBEE certificate or sworn affidavit for race, women, and youth; medical certificate from an HPCSA-registered practitioner for disability. Failure to submit proof with the bid means no preference points for specific goals are claimed. The organ of state may require substantiation of claims at any time. Fraudulent claims may lead to disqualification, recovery of costs, contract cancellation, restriction from doing business with the state for up to 10 years, and criminal prosecution.

Bidders must not be listed on the Register for Tender Defaulters or the List of Restricted Suppliers. Bidders must disclose any employment by an organ of state and any relationship with employees of the procuring institution. Bidders must disclose all CSD-registered active companies linked to all directors. Bidders must certify that the bid was prepared independently and without collusion.

1.1 Any person (natural or juristic) may make an offer or offers in terms of this invitation to bid. In line with the principles of transparency, accountability, impartiality, and ethics as enshrined in the Constitution of the Republic of South Africa, 1996 (Constitution), and further expressed in the various applicable legislation, it is required for the bidder to make this declaration in respect of the details required hereunder.

Returnable form: SBD 4 (Bidder's Disclosure) — bidders must disclose whether any director, trustee, shareholder, member or partner is employed by an organ of state, has a relationship with anyone employed by the procuring institution, or has an interest in any other related enterprise. Failure to disclose all CSD-registered active companies linked to all directors leads to disqualification. The declaration certifies the bid was prepared independently and that the bidder was not involved in drafting the specifications. A false declaration disqualifies the bid and may lead to referral to the Competition Commission or restriction from doing business with the state for up to 10 years.

Quotations must be on company letterhead, show the VAT registration number if VAT registered, and include the quotation reference number, validity period, delivery period, CSD Master registration number (MAAA) and tax compliance status PIN.

Returnable forms: SBD 4 (Declaration of Interest) and SBD 6.1 (Preference Points Claim) must be completed, signed and returned. Failure to complete SBD 4 may disqualify the quotation.

Supporting documents: certified copies (not older than 6 months) of ID, CIPC registration, B-BBEE certificate or sworn affidavit, and an e-filing copy of a valid tax clearance certificate. OEM certification documents where applicable.

Certification dates: all certified copies must be stamped by a Commissioner of Oaths within 6 months of the closing date. Documents with older or missing certification dates are rejected and the bidder scores zero for that specific goal.

Stats SA may request additional information after receipt; failure to respond within the given timeframe may render the quotation non-responsive.

If unable to quote, bidders should state reasons in writing to the same email address.

Evaluation uses the 80/20 system: 80 points for price and 20 points for specific goals and preferential points.

Specific goals and points: Black ownership 10, Women 5, Disabled 2, Youth (below 35) 3.

Points for equity ownership by an HDI are calculated using a formula based on the percentage of equity owned.

Bidders claiming specific goals must provide proof: for Black, Women, and Youth, a certified ID and company registration; for Disabled, a medical certificate from an HPCSA-registered practitioner.

Bidders must be registered on the Central Supplier Database (CSD) and have a valid tax clearance certificate.

Quotations received after the closing date and time will not be accepted.
