• SBD 4 (Bidder's Disclosure): discloses whether any director, shareholder or controlling person is employed by the state or connected to the procuring institution. False disclosure disqualifies the bid.
• SBD 6.1 (Preference Points Claim): claims specific-goal points; must be signed and supported by the required ownership evidence.
• Quotation on company letterhead, including item descriptions, validity period, banking details, contact details and CSD MAAA number.
• Food Premises Certificate issued by the Municipality in the name of the bidding company (mandatory).
• CSD Registration Report or MAAA number.

Disqualification risks:

• The items include hazardous chemicals (hydrochloric acid, sodium hydroxide, paraformaldehyde) and laboratory consumables. Bidders must ensure safe handling and delivery in accordance with applicable regulations.

• Prices must be firm until delivery; no price adjustment except for exchange rate.
• No goods/services to be delivered before an official order is received.
• Failure to deliver within agreed timelines may result in order cancellation and possible inclusion on the Database of Restricted Suppliers.
