Submit quotes/proposals and all accompanying documentation by email to [email protected] and [email protected] before 11h00 on 04 September 2026. The signed RFQ document must be included. Failure to supply any required administrative document may lead to the quote not being considered. Required documents: CSD report with supplier number and Tax Status Verification Pin; detailed pricing structure on company letterhead; valid B-BBEE certificate or sworn affidavit; signed RFQ document; valid tax certificate or tax pin; SBD 4 and SBD 6.1 forms; evidence of 3 or more similar completed assignments.

Service Providers must submit all documents as outlined in Table below: 1. Central Supplier Database Report – with supplier number and company details (www.csd.gov.za) and Tax Status Verification Pin together with tax registration number; 2. Detailed pricing structure on your company letterhead: A cost schedule detailing full cost breakdown, inclusive of VAT, any disbursement, and escalations, if applicable, etc.; 3. A Valid B-BBEE Certificate or Sworn Affidavit to determine the Service provider’s status level; 4. Signed Request for Quotation (RFQ) Document; 5. Valid Tax Certificate and/or Tax Pin; 6. SBD 4 and SBD 6.1 documentation included in this RFQ; 8. Evidence of 3 or more similar completed assignments. Note: Failure to supply any of the administrative documents stipulated may lead to the quote not being considered.

Evaluation uses the 80/20 preference point system: 80 points for price, 20 points for specific goals. Specific goals include B-BBEE status level and black ownership (50% or more black ownership = 20 points; less than 50% = 10 points; 0% = 0 points). Points for specific goals must be claimed with supporting evidence; failure to submit proof means no points are claimed. The lowest quotation will not necessarily be accepted.

Supply and deliver 3 heavy-duty melamine customised tender boxes. Specifications: made of heavy-duty melamine boards, tamper-resistant; no tools or additional materials required for use; supplied complete and finished; on wheels with brake/lock mechanism when stationary; clear tender submission instructions printed directly on surface (no loose leaflets); robust construction able to hold internal load of 30kg without distortion; stable and rigid joints; top designed to resist caving in; square shape for stacking; capable of holding 50 A4 tender files; key locking style with three spare keys; sealed compartment when in use, only openable via key; rectangular slot approximately 25cm by 6cm (tolerance not exceeding 10cm in either dimension) in the lid for depositing files; fixed wheels with stop/go/lock mechanism; closing mechanism for deposit slot (e.g., folding flap with 'Insert tender Here' and arrow) capable of being sealed. Dimensions: 800mm (length) by 650mm (width) by 1.5m (height) with tolerance of 1cm in either dimension.

Bidders may develop and submit designs within the given specifications. Bidders must ensure their submissions do not infringe on third-party rights; the bidder will be held liable for any infringement. Non-disclosure agreements will be finalised during contracting. The successful service provider must supply and deliver 3 heavy-duty melamine customised tender boxes according to the detailed specifications, including dimensions, load capacity, locking mechanisms, and printed instructions.

Pricing must be submitted on the provided schedule: item description (supply and delivery of 3 heavy-duty melamine customised tender boxes), quantity, subtotal, VAT (15%), and total bid price. The total evaluated bid price will be used for price points under the 80/20 system. Quotes valid for 90 days. Payment may be withheld until deliverables are accepted. A breakdown of assumptions, resource categories, and rates must be attached. No variation after award.

Provide a full cost for the described service, including any incidentals such as travel costs, equipment, hardware, inclusive of VAT. Pricing schedule must include item description, quantity, subtotal, VAT (15%), and total bid price. The Total Evaluated Bid Price will be used to calculate price points under the 80/20 system. Quotes must be valid for 90 days from the bid closing date. Iziko may withhold payment until a deliverable is submitted and accepted as complete by the project manager. Bidders must attach a breakdown of assumptions, resource categories, and rates. A formal detailed quotation must be sent with the proposal; no variation after award.

B-BBEE certificate or sworn affidavit required. SBD 4 (Declaration of Interest) and SBD 6.1 (Preference Points Claim Form) must be completed and signed. Evidence of 3 or more similar completed assignments required. Bidders must not be listed on the Register for Tender Defaulters or List of Restricted Suppliers. Failure to supply any required administrative document may lead to disqualification.

Bidders must confirm no conflict of interest, no association with other tendering entities, and no conviction for fraud or corruption in the last five years. False declarations may lead to disqualification, cancellation of contract, recovery of costs, and restriction from doing business with the state for up to 10 years.
