Returnable documents: SBD 4 (Bidder's Disclosure) must be completed, signed, and submitted with the quotation. The form requires disclosure of any employment by an organ of state, relationships with procuring institution staff, and all CSD-registered active companies linked to directors. Failure to disclose all such companies leads to disqualification.

Bidders must not be listed in the Register for Tender Defaulters or the List of Restricted Suppliers. Bidders must certify that the bid is independent and not collusive. Bidders must disclose all CSD-registered active companies linked to all directors; failure to do so leads to disqualification.

1.1 Any person (natural or juristic) may make an offer or offers in terms of this invitation to bid. In line with the principles of transparency, accountability, impartiality, and ethics as enshrined in the Constitution of the Republic of South Africa, 1996 (Constitution), and further expressed in the various applicable legislation, it is required for the bidder to make this declaration in respect of the details required hereunder.

Returnable documents — all must be completed, signed and submitted with the RFQ/RFP:

• Sworn Affidavit for EME/QSE (Annexure B): must be completed and returned for SMME preference points to be allocated.

SMME preference points are only allocated to bidders that are an EME (annual total revenue R10,000,000.00 or less) or a QSE (annual total revenue between R10,000,000.00 and R50,000,000.00). The sworn affidavit must be completed and returned with the RFQ/RFP to qualify.

Bidders must complete and submit the sworn affidavit (Annexure B) declaring status as EME or QSE, including Black Beneficiary percentages and total revenue for the latest financial year. The affidavit must be sworn before a Commissioner of Oaths and is valid for 12 months from the date signed.
