Bidders must declare any relationships with state employees or conflicts of interest (SBD4). • The service provider will be appointed for hosting, maintenance, support, and enhancement of the existing District Health Information System (DHIS).
• The contract period is three (3) years.
• The scope includes ensuring the system conforms to standards mentioned in the bidding documents and specifications.
• The supplier must maintain confidentiality of contract documents and information and permit inspections and audits as required.

• The supplier must not disclose contract documents or information without prior written consent from the purchaser.
