Returnable documents (all must be completed, signed and submitted on the original municipal forms): - mbd 1 (invitation to bid): bidder details and signed offer cover. - Mbd 4 (declaration of interest): discloses any director or shareholder employed by the state or connected to the municipality. - Mbd 6.1 (Preference points claim): claims b-bbee points under the 80/20 system. - Mbd 6.2 (Declaration for local production and content): confirms 100% local content. - Mbd 6.4 (Declaration of bidder's past SCM practices). - Mbd 8 (declaration of bidder's past supply chain management practices). - Mbd 9 (certificate of independent bid determination): certifies NO collusion or price disclosure. - Cipc registration documents and certified ID copies of directors (not older than 3 months). - Proof of csd registration. - Valid SARS tax pin printout. - Signed certification that NO undisputed municipal accounts are overdue by more than 30 days, with proof. - Financial statements for the past three years. - For joint ventures: valid tax pins for all partners and a signed agreement naming the lead partner. Disqualification risks: - any returnable document omitted or incomplete. - Unsigned alterations to the boq, quotation or pricing schedule. - Bids not clearly marked with the project name and reference number. - Bids over r2.3 Million from bidders not registered for VAT before closing. - Failure to initial every page of the tender document. Validity — bids must remain valid and irrevocable for 90 days from closing, extendable in writing.
