Returnable documents: signed 37.2 Agreement in terms of the occupational health and safety act (1993) acknowledging responsibilities as the service provider. Must include: (1) authorised representative declaration, (2) compliance with OHS act and ethekwini municipality site rules, (3) commitment to inform municipality of inability to perform, (4) health and safety file for approval by OHS unit, (5) activity risk assessments, written safe working procedures, and employee training records, (6) vehicle safety compliance, (7) valid medical certificates of fitness for all employees, (8) PPE compliance for all employees, (9) working at heights precautions, (10) permit to work from building supervisor before commencing work, (11) registration with compensation for occupational injuries and diseases commissioner, (12) familiarity with ethekwini municipality emergency evacuation plan.
