Submission method: deposit the completed bid document, clearly marked with the bid number and description, into the tender box at the reception area of the uMzinyathi District Municipality Offices, 39 Victoria Street, Princess Magogo Building, Dundee, 3000, before the closing time. Bids sent by courier must also be deposited in the tender box, not handed to staff. Telegraphic, faxed, or emailed bids will not be accepted. Bids must be submitted on the official bidding documents (not re-typed). Bidders must initial every page. The quotation must be on the company letterhead.

Returnable documents (all must be completed, signed, and submitted):

• MBD 1 (Invitation to Bid): completed and signed as the offer cover page.
• MBD 3.1 (Pricing Schedule – Firm Prices): completed, stamped with company stamp, and signed.
• MBD 4 (Declaration of Interest): discloses any connection to persons in the service of the state; all companies involved as per the CSD report must be listed in paragraph 3.14.
• MBD 6.1 (Preference Points Claim): claims preference points under the 80/20 system.
• MBD 8 (Declaration of Past Supply Chain Management Practices): declares past SCM practices.
• MBD 9 (Certificate of Independent Bid Determination): certifies the price was set independently, with no collusion.
• Original certified copy of the owner's identity document (not older than six months).
• Proof of company registration (CIPC) or a copy of the identity document if a sole proprietor.
• Recent municipal account (rates and taxes or municipal services) in the name of the bidder, not owing municipal services for more than 90 days, or acceptable proof of residence with a certified affidavit or lease agreement.
• Recent municipal account in the name of the director(s) of the company, not owing municipal services for more than 90 days, or acceptable proof of residence with a certified affidavit or lease agreement.
• Full CSD report.
• Original valid tax clearance certificate.

Disqualification risks:

• Any compulsory document omitted or not completed in its entirety.
• False declaration on paragraph 3.14 of MBD 4 or false information on the CSD report.
• Bid received after the closing time.
• Bid not submitted on the official forms.
• Bid signed by a person without authority to sign.
• Bidder in the service of the state.

Evaluation is conducted in terms of the Council Supply Chain Management Policy using the 80/20 preferential point system.

Scoring split:

• Price: 80 points
• Specific goals: 20 points
• Total: 100 points

Specific goals (20 points) are allocated as follows:

• Enterprises that are 100% black owned: 10 points (evidence: CIPC registration documents or detailed CSD report or copy of RSA identity documents for director(s)).
• Locality (within district): 10 points (evidence: preferred address on CSD report and utility bill for property rates and services, or original signed letter from the chief or local leader (induna), or original proof of residential signed by a ward councillor not later than 3 months).
• Within province: 5 points.
• Outside province: 2 points.

Bidders must be responsive (all compulsory documents submitted and completed) to be considered for the functionality stage. The municipality is not bound to accept the lowest or any quote.

Supply and delivery of 50 plastic sheets for disaster relief.

Delivery costs must be included in the bid price. Delivery destination is as prescribed in the contract.

Variation orders: if the estimated value of changes does not vary more than 15% of the total contract value, the contractor may be instructed to deliver the goods or render services as such. For measurable quantities, the contractor may be approached to reduce the unit price, provided there is no escalation in price.

Goods supplied shall conform to standards mentioned in bidding documents and specifications. Inspections, tests and analyses may be carried out; goods not complying may be rejected.

Pricing schedule: 50 plastic sheets, 4m x 30x250 micron SABS approved, any colour. Sub-total, VAT (15%), total. All delivery costs must be included in bid price. Prices must be valid for at least 90 days from date of offer, firm and inclusive of VAT.

Pricing format: firm prices, inclusive of VAT and disbursements. Prices must be quoted in South African Rand.

Pricing schedule (MBD 3.1):

• Description: Plastic Sheets, quantity 50, 4m x 30x250 micron SABS approved, any colour.
• Sub-total, VAT (15%), and total must be shown.

Price validity: quoted prices must remain valid for at least 90 days from the date of the offer.

Payment: payments will be made within 30 days after submission of a valid invoice, in accordance with GCC clause 16.3. Payment will be in Rand.

Performance security: the successful bidder must furnish performance security within 30 days of notification of contract award, in the form of a bank guarantee, irrevocable letter of credit, or cashier's/certified cheque, as specified in the SCC.

Taxes: the supplier is responsible for all taxes, duties, and levies until delivery. No contract will be concluded with a bidder whose tax matters are not in order or whose municipal rates and services charges are in arrears.

CSD registration: bidders must be registered on the Central Supplier Database (CSD) and submit a full CSD report.

Tax compliance: bidders must provide a valid tax clearance certificate or a SARS TCS PIN / CSD number. Foreign suppliers must complete the pre-award questionnaire in Part B:3.

Municipal account: bidders and their directors must not owe municipal services for more than 90 days, or provide acceptable proof of residence with a certified affidavit or lease agreement.

State service: no bids will be considered from persons in the service of the state.

Company registration: proof of CIPC registration or a copy of the identity document for sole proprietors.

Identity: original certified copy of the owner's identity document (not older than six months).

B-BBEE: preference points claimed under the 80/20 system via MBD 6.1. Specific goals include 100% black-owned enterprises (10 points) and locality (within district 10 points, within province 5 points, outside province 2 points).

General Conditions of Contract: definitions, application, general, standards, use of contract documents and information inspection, patent rights, performance security, inspections/tests/analyses, packing, delivery and documents, insurance, transportation, incidental services, spare parts, warranty, payment, prices, variation orders, assignment, subcontracts, delays in supplier's performance, penalties, termination for default, anti-dumping and countervailing duties and rights, force majeure, termination for insolvency, settlement of disputes, limitation of liability, governing language, applicable law, notices, taxes and duties, transfer of contracts, amendment of contracts, prohibition of restrictive practices.

Special conditions of bid: closing date and time for submission; no bid received after closing date will be considered; bids must remain valid for 90 days from closing date; bidders must initial every page; no bid shall be considered unless submitted on attached bidding documents; failure to complete forms in every aspect may invalidate the bid; all prices must be in South African currency; bidders to familiarize themselves with conditions of payment as laid down in point 16.3 of GCC; bidders will not be informed whether successful, but name of successful bidder will be published on municipal website.
