• Submit electronically via the national treasury etender portal (www.etenders.gov.za). NO physical, email, fax, courier, or late submissions accepted. • Clearly mark your submission: “request for tender from abt panel of service providers SCMU3-23/24-0048 for the appointment of a service provider to develop and construct cacadu abt clinic”. • Upload all parts of the tender offer before the closing time: 11:00am on 21 august 2026. Late bids will not be accepted. • You must be registered on the central supplier database (csd) and provide your csd number; verification will be done electronically. • Required returnable documents include: sbd1 part a & b, compulsory enterprise questionnaire, sbd4, sbd6.1, CIDB certificate, csd report, part c1.1 Form of offer and acceptance, part c2.2 Bill of quantities, resolution for signatory, certificate of authority for joint ventures (if applicable), and proof of specific goals claimed. • Failure to submit mandatory documents will result in elimination. • Tender validity period is 90 days. • For e-submission difficulties, contact 021 406 9229 / 012 406 9222 or email [email protected].

Sealed bids must be endorsed with the bid description and bid number, and deposited in the tender box at the entrance of peace initiative hall. Completed documents must comply with the conditions of tender and be submitted before the closing time. Returnable forms: the standard national treasury forms (sbd/mbd) included in the tender pack must be completed, signed, and submitted as part of the bid. These include the invitation to bid (sbd 1), pricing schedule (sbd 3), declaration of interest (sbd 4), preference points claim (sbd 6.1), Declaration for local production and content (sbd 6.2), Contract form (sbd 7), declaration of past SCM practices (sbd 8), certificate of independent bid determination (sbd 9), and any authority to sign or board resolution. Each form commits the bidder to the corresponding declaration or undertaking; failure to submit any required form may disqualify the bid.
